Incident reporting procedure

All incidents — including injuries, misconduct, equipment failure, or unsafe conditions — must be documented to ensure accountability and improve safety.

How to Report an Incident:

Notify a board member, referee, or captain immediately.

Complete the MASHA Incident Report Form as soon as possible.

The form includes:

Date, time, and location of the incident

Names and roles of the individuals involved

A detailed description of what occurred

Any witnesses

Actions taken (e.g. first aid, game stoppage)

Where Reports Go:

Submissions are sent directly to the MASHA board and stored securely.

You’ll receive a confirmation email after submission.

Follow-Up:

Serious incidents may require review or policy updates.

Trends will be reviewed quarterly to inform future safety decisions.
