I acknowledge that participation in any activities, events, or programs hosted or associated with the Metro Atlanta Street Hockey Association ("MASHA") involves inherent risks, including but not limited to, the risk of personal injury, permanent disability, property damage, or death. These risks may arise from my own actions, the actions or inactions of others, or the conditions of the facilities or equipment used.
I understand and accept full responsibility for all such risks, both known and unknown. I voluntarily assume all risks related to participation in MASHA events at Pinckneyville Park (a Gwinnett County facility), whether I am participating as a player, volunteer, or in any other capacity.
In consideration for being allowed to participate in MASHA activities, I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless MASHA, its board members, volunteers, sponsors, and agents; Pinckneyville Park; Gwinnett County; and all respective officials, employees, and representatives from any and all claims for injury, disability, death, loss, or damage to person or property, whether arising from the negligence of the released parties or otherwise, to the fullest extent permitted by law.
I agree to indemnify and hold harmless MASHA, its board members, officers, volunteers, agents, sponsors, Pinckneyville Park, Gwinnett County, and all affiliated individuals and entities from any and all claims, liabilities, damages, or expenses (including attorney's fees) arising out of or resulting from my participation (or my child's participation) in MASHA activities. This includes, but is not limited to, claims arising from my own actions, the actions of others, or from any breach of this agreement.
If the participant is under eighteen (18) years of age, I affirm that I am the parent or legal guardian and I consent to this waiver on their behalf. I agree that all terms herein apply equally to the minor child.
I acknowledge that hockey is a physical sport and carries the risk of injury, including concussions or other head trauma. I understand the signs and symptoms of a concussion, such as dizziness, confusion, headache, nausea, or memory issues, and agree to promptly report any symptoms of injury. I will remove myself (or my child) from play if it is unsafe to continue and will comply with MASHA's return-to-play policy, which may require written clearance from a licensed healthcare professional before returning to participation.
In the event of a medical emergency, I authorize MASHA representatives to provide or seek appropriate medical attention for me (or my child, if under 18). I understand that I am responsible for any resulting medical expenses.
I understand that MASHA reserves the right to modify schedules or cancel scrimmages due to weather, safety concerns, insufficient attendance (minimum 15 players required), or other circumstances. All fees are non-refundable, including in the event of cancelled scrimmages due to low attendance, holidays, or weather. No credits or make-up sessions are guaranteed.
All participants must demonstrate good sportsmanship at all times. Unsportsmanlike conduct will not be tolerated during any MASHA event, including scrimmages or informal pick-up play. This includes, but is not limited to:
Derogatory or discriminatory remarks or behavior, including racist, homophobic, or transphobic language
Verbal abuse or excessive arguing with other players
Malicious or excessive profanity, insults, or gestures
Taunting, baiting, or instigating fights with other players
Intentional or dangerous physical contact, including spitting or any action deemed unnecessarily rough
Abuse, threats, or harassment directed at volunteers or MASHA staff
Any conduct determined by the MASHA Board to be harmful to the safety, integrity, or spirit of the league
Violations may result in suspension or permanent removal from the league at the discretion of the MASHA Board.
By participating in MASHA, I agree to abide by all rules outlined in the Official MASHA Rulebook. I understand these rules are intended to ensure fair play, safety, and a positive community environment. I also acknowledge that MASHA reserves the right to update or amend its rules or policies as needed.
I authorize MASHA and its representatives to take and use photographs or video footage of me (or my child, if under 18) for promotional, educational, and marketing purposes, with or without the use of names.
MASHA collects personal information for team placement, communication, and league management. This information is handled in accordance with our Privacy Policy, available at mashahockey.com/privacy-policy. By submitting this form, I consent to receive email communications from MASHA related to league operations and events. I understand I can unsubscribe at any time.
By completing and submitting this form, I acknowledge that I have read and understood all sections above. I understand that I am giving up substantial legal rights by submitting this form, and I do so voluntarily and without coercion.
This agreement is valid for the duration of my participation in MASHA, including all scrimmages, pick-up play, and related activities, and remains in effect until I notify MASHA in writing that I am withdrawing from the league.
If the participant is under 18 years of age, I confirm that I am their parent or legal guardian and agree to these terms on their behalf.
