Liability Waiver & Release of Liability

PLEASE READ CAREFULLY – THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS

Assumption of Risk and Release of Liability

I acknowledge that participation in any activities, events, or programs hosted or associated with the Metro Atlanta Street Hockey Association (“MASHA”) involves inherent risks, including but not limited to, the risk of personal injury, permanent disability, property damage, or death. These risks may arise from my own actions, the actions or inactions of others, or the conditions of the facilities or equipment used.

I understand and accept full responsibility for all such risks, both known and unknown. I voluntarily assume all risks related to participation in MASHA events at Pinckneyville Park (a Gwinnett County facility), whether I am participating as a player, volunteer, or in any other capacity.

In consideration for being allowed to participate in MASHA activities, I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless MASHA, its board members, volunteers, sponsors, and agents; Pinckneyville Park; Gwinnett County; and all respective officials, employees, and representatives from any and all claims for injury, disability, death, loss, or damage to person or property, whether arising from the negligence of the released parties or otherwise, to the fullest extent permitted by law.

Indemnification

I agree to indemnify and hold harmless MASHA, its board members, officers, volunteers, agents, sponsors, Pinckneyville Park, Gwinnett County, and all affiliated individuals and entities from any and all claims, liabilities, damages, or expenses (including attorney’s fees) arising out of or resulting from my participation (or my child’s participation) in MASHA activities.

Minor Participants

If the participant is under eighteen (18) years of age, I affirm that I am the parent or legal guardian and I consent to this waiver on their behalf.

Concussion and Injury Awareness

I acknowledge that hockey is a physical sport and carries the risk of injury, including concussions or other head trauma. I understand the signs and symptoms of a concussion and agree to promptly report any symptoms of injury.

Medical Treatment Authorization

In the event of a medical emergency, I authorize MASHA representatives to provide or seek appropriate medical attention for me (or my child, if under 18). I understand that I am responsible for any resulting medical expenses.

Participation Disclaimer

I understand that MASHA reserves the right to modify schedules or cancel scrimmages due to weather, safety concerns, insufficient attendance (minimum 15 players required), or other circumstances. All fees are non-refundable.

Code of Conduct

All participants must demonstrate good sportsmanship at all times. Unsportsmanlike conduct will not be tolerated, including derogatory remarks, verbal abuse, malicious contact, taunting, or harassment. Violations may result in suspension or permanent removal from the league.

Photo/Video Release

I authorize MASHA and its representatives to take and use photographs or video footage of me (or my child, if under 18) for promotional, educational, and marketing purposes.

Privacy Policy

MASHA collects personal information for team placement, communication, and league management. This information is handled in accordance with our Privacy Policy, available at mashahockey.com/privacy-policy.

Acknowledgment and Agreement

By completing and submitting this form, I acknowledge that I have read and understood all sections above. I understand that I am giving up substantial legal rights by submitting this form, and I do so voluntarily and without coercion. This agreement is valid for the duration of my participation in MASHA.
